Achtergrond Ehlers Danlos Ogen

Ehlers Danlos Ogen komen voort uit het Ehlers Danlos Syndroom. Er zijn verschillende types die behoren tot de groep erfelijke bindweefselafwijkingen, waarvan de oorzaak ligt in het genetisch materiaal. De kenmerken zijn over het algemeen de hypermobiliteit van de gewrichten, een rekbare en elastische huid (hyperextensibiliteit) en kwetsbaarheid van het weefsel. Er zijn 14 subtypes bekend waarvan de genetische oorzaak varieert en waarbij elk type weer zijn eigen kenmerken heeft die tot uiting komen. Het meest voorkomende subtype is hypermobiele Ehlers Danlos Syndroom (hEDS).

Waar Ehlers Danlos Ogen uit bestaan

Het lichaam bestaat voor het grootste gedeelte uit bindweefsel zoals: ligamenten, pezen en spieren van het bewegingsstelsel, het hart en bloedvatenstelsel, organen van bijv. het spijsverteringsstelsel, de huid dat het grootste orgaan van het menselijk lichaam is, het tandvlees en je zintuigen zoals het oog. Het oog bestaat voor ca. 80% uit bindweefsel. Daarom verbaast het niet dat mensen met het Ehlers Danlos Syndroom regelmatig problemen met de ogen hebben, zoals visusklachten, diplopie en droge ogen, en dat de kans op een oogziekte aanzienlijk groter is dan normaal gesproken.

De diagnose Ehlers Danlos Syndroom (of HSD) zorgt ervoor dat het bindweefsel extra kwetsbaar is. Daarom is het noodzakelijk om deze aspecten zorgvuldig te controleren.

Controle oogleden en traanapparaat

De controle van het voorsegment van Ehelrs Danlos Ogen bestaat uit het volgende:

  • Dry eye syndrome met traanfilminstabiliteit
  • Floppy eyelid syndroom, zowel lateraal als bilateraal
  • Snel geïrriteerde oogleden en slijmvliezen (frequenter bij MCAS)
  • Epicanthal plooien (Vaak gezien bij kinderen, en soms ook volwassenen, met  cEDS)

Controle achtersegment Ehlers Danlos Ogen

  • Glasvochtafwijkingen
  • Angioïde strepen
  • Netvliesloslating
  • Verhoogde oogdruk/ Glaucoom
  • Retinal holes en tears
  • Zwakke plekken aan de randen van het netvlies
  • Maculadegeneratie
  • Spontane carotis-caverneuze sinusfistels (sCCF)   komt vaker voor bij vEDS

Veelvoorkomende problemen met het zien en de samenwerking van de Ehlers Danlos Ogen én regelmatig in combinatie met OI/ dysautonomie als extra oorzaak. Wat bespreek ik met de orthoptist, optometrist of (medische) opticien-contactlensspecialist?

  • Refractieafwijkingen: voornamelijk (pathologische) myopie en astigmatisme
  • Ernstige hypermetropie of uitpuilende ogen (gezien bij spEDS en zijn beide genspecifiek)
  • Accommodatie insufficiëntie: o.a. accommodatiespasme en pre-presbyopie
  • Convergentie insufficiëntie
  • Motiliteitsstoornissen en diplopie- Strabisme en phorieën

Veelvoorkomende klachten die een patiënt met EDS kan ervaren, waarbij OI/ dysautonomie een rol kan spelen:

  • Wazig zien en wisseling in visus
  • Dubbelzien en last van schaduw, bijv. bij lezen
  • Fotofobie
  • Stipjes, draadjes, spinneragjes die meegaan met de oogbeweging
  • Hoofdpijn: voornamelijk frontaal boven de ogen en temporaal bij de slapen
  • Migraine met aura, migraine zonder aura, spanningshoofdpijn, combinatie migraine en spanningshoofdpijn
  • Algehele malaise
  • Droog of zanderig gevoel van de ogen

Belangrijke informatie om te weten bij keuze anesthesie, chirurgische behandeling en nazorg van iemand met het Ehlers Danlos Syndroom.

  • Ik ben geen kandidaat voor LASIK of radial-K. Ook ben ik gevoeliger voor keratectasia  na de   behandeling.
  • Er is een grotere kans op complicaties na een oogheelkundig chirurgische ingreep.- Er kan sprake zijn van een verhoogde mate van lokale anesthetische resistentie.

Conclusie Ehlers Danlos Ogen

Een oftalmologische en orthoptische evaluatie moet worden uitgevoerd bij alle patiënten met het Ehlers Danlos syndroom om Ehlers Danlos Ogen op te sporen en te behandelen. Als het Ehlers-Danlos syndroom wordt vermoed, kan oogheelkundig onderzoek ook ondersteuning bieden voor de diagnose.
